It is vital we publicise St Wilfrid’s work to help our community understand more about hospice care, and to encourage our community to support our charity. St Wilfrid’s is funded through a small grant from the NHS, and we rely on the support of the public for the remainder of our over £9m per year running costs. By sharing your story, you are making a big difference to the work of our charity and we greatly appreciate your support.

To comply with the Data Protection Act 1998 and General Data Protection Regulations we need to seek your consent before we take/use any interview material or photographs of you.

“I give my consent for photographs, audio and or/film recordings and interview material featuring me obtained by St Wilfrid’s to be used to promote the work of St Wilfrid’s Hospice.

I understand this material will be stored securely for future use in perpetuity in all media unless specified otherwise. I understand I can withdraw my consent at any time by emailing marketing@stwh.co.uk or calling the Hospice on 01243 775302. I understand that any images or words attributed to me may continue to appear in print and digital publications already in circulation/the public domain.

I understand St Wilfrid’s may use my photographs, recordings or case studies in Hospice publicity, e.g. St Wilfrid’s newsletter, posters and leaflets; on the Hospice website www.stwh.co.uk; on Hospice social media accounts or other electronic media such as Facebook, Twitter, Instagram, YouTube, and press releases, which may subsequently appear in partner publications and local or national media.

I understand that once my photo/words are published and in circulation (for example in newspapers or on the internet) they may be copied and used by others.”

We will not include your full name in an image on our website, video or in printed publications, without good reason or prior agreement

We will never include your e-mail, postal address or telephone number, without your prior consent

We will not use the images taken, or any other information you provide, for any other purpose other than to promote the work of St Wilfrid’s Hospice
