Price: £300 for the full course per care home (two senior representatives per care home), including all course materials. This course is heavily subsidised by West Sussex Clinical Commissioning Group and includes, our Practice Educator (for Care Homes) support.
Where: Virtual via Microsoft Teams (other than the first workshop which we hope to hold face to face at the Education Centre depending on government guidelines at the time).
NOTE: Places are limited to 10 Care Homes in West Sussex (two cohorts of 5 Care Homes). If you would like your care home to be considered for a place, please complete the application request to register your interest.
The Six Steps+ Programme is a series of nine workshops designed to provide care homes within West Sussex with a toolkit that is consistent with current End of Life Care guidance.
The Six Steps programme is a national programme, originally created by the Cumbria and Lancashire End of Life Care Network. It was further developed into the Six Steps+ Programme by St Luke’s Hospice in Plymouth to include a dementia workshop, focusing on end of life care for those with dementia.
It consists of a series of nine 6 hour workshops (09.00am—15.00pm), which runs over 8 months, followed by a 1 month consolidation period to allow embedding of learning in the workplace, using a whole organisational approach. Portfolios of evidence are developed to demonstrate achievement of assessment criteria (consistent with CQC Fundamental Standards and current national End of Life Care guidance).
The potential benefits to residents and to the organisation of undertaking the Six Steps+ programme make the significant commitment involved highly worthwhile.
Each participating home will be asked to send two senior representatives (NVQ/QCF Level 3 or above). Ideally, in the first instance one of the representatives should be a Care Home Manager or Care Manager.
