Physiotherapist - Maternity Cover (up to 6 months)

An opportunity for Physiotherapist to join the Living Well Services team has arisen to further develop our specialist palliative respiratory and neurological services to patients and families within the community we serve.
This is a great opportunity for a dynamic and experienced individual to provide a high quality and efficient Physiotherapy service to patients with a wide range of physical and medical conditions referred to the Specialist Palliative Care Service. The new hospice has 4 treatment rooms, and indoor gym facilities, to support this service.
The successful applicant will be required to work as an integral member of the multidisciplinary team, promoting a high quality rehabilitative palliative care service that integrates rehabilitation, enablement, self-management and self-care into the holistic model of palliative care to patients in all care settings of the service. This includes the Inpatient Unit, Living Well Services and the Community.
The successful candidate must have a least 3 years post registration experience with some in depth working knowledge of respiratory and neurological conditions. Hold a valid, clean driving license and use of own vehicle is essential. This position is subject to an enhanced DBS check.

- To deliver a high quality rehabilitative palliative care service that integrates rehabilitation, enablement, self-management and self-care into the holistic model of palliative care to those patients who are an outpatient, attending day hospice, in their own home or admitted to the ward
- Provides specialist programmes of care demonstrating evidence based clinical reasoning and decision-making.
- Effective caseload management to ensure that the needs of patients and their families, are met using available resources
- Works as a supportive member of the Rehabilitative and Enablement Care team; with Occupational Therapist, Nurses and PT/OT therapy assistan
- Clinical – Provide Specialist Palliative Care services for patients and carers
- People – Responsible for day to day supervision of team members including Staff and Volunteers
- Financial – Assist in monitoring departmental budgets
KEY ACCOUNTABILITIES & RESPONSIBILITIES
- Delivers a high quality Palliative Care service to patients and their carers/families, ensuring that care is planned following individual assessments and implemented and evaluated regularly.
- Work collaboratively with the patient, their relatives and carers to support patients’ to achieve their personal priorities.
- Liaises with all members of the multidisciplinary team to ensure smooth working across teams to maintain effective service provision.
- Provides continuity and support with group work and promotes feedback from service users.
- Encouraged all to identify commonly used words and phrases which may detract from a more rehabilitative approach to palliative care.
- Ensures the maintenance of a supportive atmosphere maintaining good morale.
- Works closely with members of senior staff group to maintain effective communication.
- Liaises with colleagues in other organisations as required.
- Deputises as team leader to ensure patient needs are met and a safe environment is maintained.
- Reports on complaints, untoward incidents and faulty equipment to Line Manager and/ Director of Nursing.
- Maintain a cost-effective service within agreed budget.
- Identifies and proposes effective policies and working practices within the team ensuring these are clearly understood when implemented
- Reviews work performance of others and undertake appraisals of Assistants
- Supports research based practice and the maintenance of a supportive learning environment.
- Carries out training and competency based assessments.
- Participates in Hospice Audit Programme, attends ward and clinical meetings as required.
- Membership of Professional Organisation and practices in accordance with professional code of practice.
- Allocates work to the Physiotherapy/Occupational Therapy Assistant in liaison with the Occupational Therapist.
- Ensures continuity of service through appropriate delegation to external rehab teams.

KNOWLEDGE & SPECIFIC JOB SKILLS
- Diploma / degree in Physiotherapy
- Minimum 3 years post qualifying
