Clinical Nurse Specialist

Salary: In the region of £39,461, pro rata (plus enhancements)
Hours: Full Time or Part Time hours available

We are looking for an enthusiastic, motivated individual with experience and knowledge of caring for patients with a palliative diagnosis. The post holder must possess excellent communication and interpersonal skills and have the ability to provide high quality standards of care for patients and their families at all times, through innovative service delivery. This role requires a dynamic, experienced and confident nurse who can demonstrate excellent clinical skills.

The post holder will need to have a high degree of professional autonomy and be able to make critical judgements using expert knowledge, skills and experience in order to provide the highest standard of holistic care including complex symptom management to patients at the end of life. Preferably a Non- Medical Prescriber or willingness to undertake relevant qualifications and have a diploma or degree relevant to palliative care and at least three years working as a clinical nurse specialist.

The successful candidate must be prepared to work flexibly with days including the occasional weekend work and to participate in an on call rota. In addition to this the post holder must hold a valid, clean driving licence and use of own vehicle is essential.

JOB PURPOSE

  • To provide specialist palliative care to patients and families in the community, working closely with the Primary Health Care Teams
  • Nurse prescriber

JOB SCOPE

  • People – no direct line reports but supervises and teaches other staff and students
  • To carry a caseload of up to 45 active and deferred patients
  • To liaise with specific GP practices in designated areas and undertake GSF at those practices
  • To undertake on- call duties as per rota

KEY ACCOUNTABILITIES & RESPONSIBILITIES

  • Promotes the St. Wilfrid’s Hospice culture of excellence in patient and people care
  • Contributes to reporting of progress towards meeting relevant strategic and operational goals
  • Delivers a high standard of care to patients and their carers / families, ensure that care is planned following individual assessments and implemented and evaluated regularly.
  • Undertake week-end working as per team rota
  • To undertake on-call duties for the Palliative care Hub as per SWH rota
  • Liaises with all members of the multidisciplinary team to ensure good communication about patients on the caseload; plan and attend case conferences and clinical meetings as required  
  • Undertakes role of triage nurse and attends daily referral meeting in rotation with other CNS team members when acting as triage nurse
  • Completes initial assessments of patients referred to the hospice services
  • Acts as Nurse Independent Prescriber
  • Acts as mentor for CNS secondments from other clinical areas as appropriate
  • Provides bereavement support in accordance with bereavement policy and in conjunction with Psychosocial Team
  • Manages active and deferred patient caseload, ensuring that contacts and visits are arranged according to individual need and that patients no longer requiring specialist care are discharged
  • Keeps accurate and up to date records on care and treatment given via patient electronic database ( S1)
  • Attends Gold Standard Framework meetings at designated surgeries
  • Undertakes teaching to include Mentoring and supervision of students as required
  • Communicates clearly and regularly with their CNS team leader and LNCS
  • Challenges the department/service to make continuous improvement
  • Stretches and develops the capability of the team through support and guidance
  • Contributes towards ensuring that the Hospice’s exposure to risk is minimised through safe practice and awareness of health and safety and infection control
  • Promotes research based practice and the maintenance of a supportive learning environment
  • Participates in Hospice Audit Programme, attends ward and clinical meetings as required
  • Maintains and develops effective communication with relevant organizations
  • Practices in accordance with the NMC code and revalidation
  • Motivates and directs both staff and volunteers to ensure high standards of performance and delivery
  • Provides development training and support for self and others including mandatory updates
  • Works within agreed hospice policies, procedures and guidelines
  • Volunteers are a vital resource to St. Wilfrid’s Hospice and it is the responsibility of all staff to treat them with respect and value their contribution

KNOWLEDGE & SPECIFIC JOB SKILLS
