Associate Practitioner in Community Specialist Palliative Care

We are looking for an enthusiastic and motivated Registered Health Care Professional with experience and knowledge of caring for patients with a palliative diagnosis. The post holder must possess excellent communication and interpersonal skills and have the ability to provide high quality standards of care for patients and their families at all times. This role requires a dynamic, experienced and confident nurse who can demonstrate excellent clinical skills.
The post holder will work towards a high degree of professional autonomy using expert knowledge, skills and experience in order to provide the highest standard of care including complex symptom management to patients at the end of life. Preferably holding a diploma or degree relevant to palliative care and at least five years post registration experience. You will undertake a competency - based programme working closely with the Community Clinical Nurse Specialists and your allocated mentor.
The successful candidate must be prepared to work flexibly with days including the occasional weekend work. In addition to this the post holder must hold a valid, clean driving licence and use of own vehicle is essential.
