Did you know that at any one time, 95% of our patients receive care in the community; in their own home, residential or nursing home, or in hospital?

The services we provide in the community allow people to receive our specialist care wherever they are. We offer them the practical help and support to achieve the best quality of life for as long as possible.

St Wilfrid’s Clinical Nurse Specialists

Patients are not defined by their illness but by who they are - and our team of specialist trained nurses provide care, advice and support on an individually tailored basis. This gives patients and their families comfort, peace of mind and hope – they know they are not alone.

After referral, you will be assigned a dedicated St Wilfrid’s Clinical Nurse Specialist who will be your first point of contact. Your Clinical Nurse Specialist will spend time with you and your family or carer to understand your individual needs and draw up a plan of ongoing care for you. Your nurse will also work alongside your GP and other healthcare professionals to ensure that your care is complete and co-ordinated.

Hospice at Home Team

Our Hospice at Home team provides comprehensive care in people’s own homes, including symptom control, respite care and emotional support. The team is very flexible and able to respond quickly to the needs of our patients and their families, thus enabling patients to have a greater choice about where they are cared for and ultimately where they die.

Our staff take time to listen and understand the needs and wishes of our patients and always try to go above and beyond to make sure everyone gets the care they deserve. We aim to make the last days of patients’ lives as supported, comfortable and dignified as they can be - hopefully leaving behind positive memories of that time for their family and friends.
