St Wilfrid's hospice welcomes referrals from other healthcare professions - see the referral guidelines for full details. We prefer referrals to be made using the referral form which can be completed online and faxed to us on 01243 538171. Referrals may also be made by letter or telephone call to 01243 755813 - such referrals should include all the detail requested on the referral form.
If a person is not in the St Wilfrid's catchment population there are also hospices in Portsmouth and Worthing and a community team covering the Midhurst area - see other local hospices for further details.
An electronic copy of the Wessex Palliative Care Handbook of clinical guidelines ("The Green Book") is available on this website, as is an opioid conversion guide to aid the conversion of one opioid to another. The hospice ward nursing team is able to provide out-of-hours telephone advice.

St Wilfrid's works closely with the PCT and other NHS colleagues to promote the implementation of the NHS End of Life Care Programme and uses tools such as the Gold Standards Framework and Liverpool Care Pathway.
